REMS Programs and Pharmacy Compliance Requirements

Understand Risk Evaluation and Mitigation Strategies (REMS) programs for the PTCB exam.

What Are REMS Programs?

Risk Evaluation and Mitigation Strategies (REMS) are FDA-required safety programs for medications with serious risks. REMS ensure that the benefits of a medication outweigh its risks by implementing special requirements beyond standard labeling. Elements can include medication guides, communication plans, and Elements to Assure Safe Use (ETASU).

Common REMS programs include iPLEDGE for isotretinoin (requiring two negative pregnancy tests and monthly monitoring), the Clozapine REMS (requiring regular ANC monitoring for agranulocytosis), and the Opioid Analgesic REMS (focused on prescriber education).

Pharmacy Technician Responsibilities

Technicians play a key role in REMS compliance by verifying patient enrollment before dispensing, checking authorization windows and lab results, distributing required medication guides, and documenting all compliance steps in the pharmacy system.

Failure to comply with REMS requirements can result in the pharmacy losing its ability to dispense those medications. Accurate record-keeping and attention to detail are essential.
