Understanding Pharmacy Claim Billing and NCPDP Codes

Learn pharmacy billing standards including NCPDP, BIN numbers, and claim submission for the PTCB exam.

NCPDP Standards

The National Council for Prescription Drug Programs (NCPDP) sets the standard for electronic pharmacy claim processing. Every pharmacy claim transmitted electronically follows NCPDP standards, which include fields for patient information, prescriber data, drug details, and billing information.

Key identifiers include the BIN (Bank Identification Number), PCN (Processor Control Number), and Group Number, which together route the claim to the correct PBM for processing. These numbers are found on the patient's insurance card.

Claim Submission Process

When a prescription is entered and submitted, the claim is transmitted electronically to the PBM in real time. The PBM adjudicates the claim, checking eligibility, formulary status, and pricing. The response comes back within seconds, showing the approved amount and patient copay.

DAW (Dispense as Written) codes indicate substitution instructions. DAW 0 means substitution is permitted, DAW 1 means the prescriber requested brand, and DAW 2 means the patient requested brand.

Exam Practice

Billing and claims processing is a core PTCB topic. Study with PTCB flashcards and take practice exams to reinforce these concepts.
