Pharmacy Benefit Managers (PBMs) Explained for PTCB

Understand the role of PBMs in pharmacy insurance processing and how they appear on the PTCB exam.

What Is a PBM?

Pharmacy Benefit Managers (PBMs) are intermediaries between insurance companies, pharmacies, and drug manufacturers. They manage prescription drug benefits for health plans by negotiating drug prices, processing claims, and creating formularies. Major PBMs include CVS Caremark, Express Scripts, and OptumRx.

PBMs determine which medications are covered, at what cost tier, and whether prior authorization is required. Understanding PBM functions is essential for pharmacy technicians who process insurance claims daily.

How PBMs Affect Pharmacy Operations

PBMs set reimbursement rates for pharmacies, often based on AWP or MAC (Maximum Allowable Cost) pricing. They also manage mail-order pharmacy programs and specialty pharmacy networks. Pharmacies must contract with PBMs to be included in their networks.

When a claim is rejected, the pharmacy technician must understand common rejection codes and know when to contact the PBM for resolution, such as when a prior authorization is needed or a patient has changed insurance.
