Medication Therapy Management: A PTCB Overview
Understand medication therapy management for the PTCB exam. Learn about MTM services, comprehensive medication reviews, and the pharmacy tech role.
What Is Medication Therapy Management?
Medication therapy management (MTM) is a group of services designed to optimize therapeutic outcomes for patients through improved medication use. Originally established by the Medicare Modernization Act of 2003, MTM programs help patients with multiple chronic conditions manage complex medication regimens. Understanding MTM is important for the PTCB certification exam.
Goals of MTM
The primary goals of MTM programs include:
- Optimizing medication therapy: Ensuring each medication is appropriate, effective, safe, and being taken correctly
- Reducing adverse drug events: Identifying and resolving drug interactions, duplicate therapies, and inappropriate medications
- Improving adherence: Helping patients understand and stick to their medication regimens
- Reducing healthcare costs: Preventing hospitalizations and emergency visits caused by medication-related problems
Core MTM Services
MTM encompasses several key services:
Comprehensive Medication Review (CMR)
A CMR is an in-depth review of all medications a patient is taking, including prescription drugs, over-the-counter medications, supplements, and herbal products. During a CMR, the pharmacist:
- Reviews the patient's complete medication list
- Identifies medication-related problems
- Develops a plan to resolve identified issues
- Provides the patient with a personal medication record and medication action plan
Targeted Medication Review (TMR)
A TMR focuses on specific medication-related concerns rather than the entire medication profile. These are typically triggered by specific events such as a new prescription, a change in therapy, or identification of a potential problem through claims data.
Personal Medication Record (PMR)
The PMR is a comprehensive list of all the patient's medications, including the drug name, dose, frequency, reason for use, and prescriber. It serves as a portable reference that patients can share with all their healthcare providers.
Medication Action Plan (MAP)
The MAP is a patient-centered document that lists specific actions the patient should take to manage their medications effectively. It includes self-management goals, follow-up dates, and what to do if problems arise.
The Pharmacy Technician's Role in MTM
While pharmacists are responsible for the clinical aspects of MTM, pharmacy technicians provide essential support:
- Scheduling MTM appointments: Identifying eligible patients and booking reviews
- Gathering patient information: Collecting medication lists, insurance information, and health history
- Preparing documentation: Organizing paperwork and records for the pharmacist's review
- Data entry: Recording MTM outcomes and interventions in the pharmacy system
- Follow-up: Contacting patients for follow-up appointments and refill reminders
Who Qualifies for MTM?
Under Medicare Part D, MTM programs target patients who have multiple chronic diseases (such as diabetes, hypertension, and heart failure), take multiple Part D medications, and are likely to incur high annual medication costs. Individual plans may have additional eligibility criteria.
