Medicare and Medicaid in Pharmacy: What Technicians Must Know

Understand the basics of Medicare Part D and Medicaid prescription coverage, formulary tiers, coverage gaps, and how pharmacy technicians process these claims.

Government Insurance Programs in Pharmacy

Medicare and Medicaid are the two largest government healthcare programs in the United States. Pharmacy technicians process claims for both programs daily, making a working knowledge of their structure essential for efficient prescription processing and PTCE preparation.

Medicare Part D

Medicare Part D is the outpatient prescription drug benefit for Medicare beneficiaries (primarily adults 65 and older). Key features:

  • Formulary tiers - Drugs are organized into tiers with different copays. Tier 1 (preferred generics) has the lowest copay, while specialty tiers have the highest.
  • Coverage gap (donut hole) - After a spending threshold, patients enter a coverage gap where they pay a higher percentage of drug costs until reaching catastrophic coverage.
  • Low-income subsidy (LIS/Extra Help) - Qualifying low-income patients receive reduced or eliminated copays and premiums.
  • Annual enrollment - Patients can change plans during the annual enrollment period (October 15 through December 7).

Medicaid

Medicaid is a joint federal-state program providing healthcare coverage to low-income individuals. Key features:

  • Coverage varies by state, as each state administers its own Medicaid program
  • Most Medicaid plans have minimal or no copays
  • Formularies and prior authorization requirements differ by state
  • Federal rebate programs help states negotiate drug prices

Dual-Eligible Patients

Some patients qualify for both Medicare and Medicaid (dual-eligible). These patients typically have Medicare as their primary prescription insurance and Medicaid as secondary. Technicians must bill both plans correctly using coordination of benefits procedures.

Processing Tips for Technicians

  • Verify the correct plan and BIN/PCN numbers for Medicare Part D plans, which change annually
  • Check formulary coverage before processing to anticipate rejections
  • Know that Medicaid is the payer of last resort and should be billed after all other insurance
  • Be aware of Medicaid preferred drug lists, which vary by state

Insurance processing is a practical skill that enhances your PTCE exam preparation and daily effectiveness.
