Troubleshooting Insurance Claim Rejections

Learn to identify and resolve common pharmacy insurance claim rejection codes for the PTCB exam.

Common Rejection Codes

Insurance claim rejections are a daily occurrence in pharmacy. Common reasons include: refill too soon (the patient is trying to fill before the allowed date), prior authorization required, drug not covered on formulary, invalid member ID, patient not found in the system, and duplicate claim.

Each rejection comes with a specific code and message. Pharmacy technicians must be able to read these messages and take appropriate action, whether that means correcting data entry, contacting the patient, or escalating to the pharmacist.

Resolution Steps

For "refill too soon" rejections, calculate when the previous fill should run out based on the day supply. For invalid member ID, verify the patient's insurance card and re-enter the information. For formulary rejections, check if a therapeutic alternative is available or initiate a prior authorization.

Coordination of benefits (COB) issues arise when patients have multiple insurance plans. The primary insurance must be billed first, then the secondary plan processes the remaining balance.
