Common Ethical Dilemmas in Pharmacy Practice

Explore real-world ethical dilemmas pharmacy technicians face. Learn how to navigate drug diversion, prescription fraud, and workplace conflicts.

Facing Ethical Challenges in the Pharmacy

Pharmacy technicians encounter ethical dilemmas more often than many people realize. From suspected drug diversion to prescription fraud, these situations require clear thinking, knowledge of policies, and a commitment to patient safety. The PTCB exam includes scenario-based questions on ethics, making this a practical and testable topic.

Drug Diversion: Recognizing and Reporting

Drug diversion occurs when controlled substances are redirected from their intended use. This can involve patients, pharmacy staff, or other healthcare workers. Signs of potential diversion include:

  • Unexplained discrepancies in controlled substance counts
  • A coworker frequently volunteering to handle controlled substances
  • Broken or missing security seals on medication containers
  • Patients visiting multiple pharmacies for the same controlled substance (doctor shopping)

If you suspect drug diversion, do not confront the individual directly. Report your concerns to your supervisor, pharmacist-in-charge, or through your organization's anonymous reporting system. Many pharmacies use Prescription Drug Monitoring Programs (PDMPs) to track controlled substance dispensing and identify suspicious patterns.

Prescription Fraud

Pharmacy technicians play a key role in detecting fraudulent prescriptions. Red flags include:

  • Prescriptions that appear altered or have inconsistent handwriting
  • Prescriber information that cannot be verified
  • Patients who are unfamiliar with their prescribed medication
  • Unusual quantities or combinations of controlled substances
  • Prescriptions from out-of-state prescribers for controlled substances

When you suspect a fraudulent prescription, alert the pharmacist immediately. The pharmacist will determine the appropriate course of action, which may include contacting the prescriber's office for verification or notifying law enforcement.

Workplace Conflicts and Error Reporting

Ethical dilemmas also arise in workplace dynamics. Common scenarios include:

  • Observing a colleague cut corners: If you see a coworker skip verification steps or bypass safety protocols, you have an ethical obligation to speak up, even if it creates tension.
  • Pressure to work too fast: High prescription volumes can create pressure to sacrifice accuracy for speed. Patient safety must always come first.
  • Disagreeing with a pharmacist: If you believe a pharmacist is making an error, express your concern respectfully. Most pharmacies have escalation procedures for unresolved disagreements.

Patient Privacy Challenges

Real-world privacy dilemmas pharmacy technicians face include:

  • A friend or family member's prescription comes through, and you are curious about their medication
  • A coworker gossips about a well-known patient's prescriptions
  • A patient's family member calls requesting prescription information
  • Social media posts that inadvertently reveal pharmacy operations

In every case, follow HIPAA guidelines and the minimum necessary standard. Only access patient information when you have a legitimate work-related need, and never share patient information without proper authorization.

Applying Ethical Decision-Making

When facing an ethical dilemma, use this framework:

  1. Identify the problem: What is the ethical issue at hand?
  2. Gather information: What policies, laws, or guidelines apply?
  3. Consider options: What are the possible courses of action?
  4. Evaluate consequences: What are the potential outcomes of each option?
  5. Act: Choose the option that best protects patient safety and upholds professional standards
  6. Reflect: After the situation is resolved, consider what you learned
