Drug Diversion Detection and Prevention in the Pharmacy

Understand drug diversion red flags, prevention strategies, and the pharmacy technician role in maintaining controlled substance integrity.

What Is Drug Diversion?

Drug diversion occurs when controlled substances are redirected from their intended medical purpose to illicit use. Pharmacy technicians are on the front lines of detection and prevention. This topic appears frequently on the PTCB exam under federal requirements.

Common Red Flags

Prescription Red Flags

  • Prescriptions from distant prescribers with no local connection
  • Multiple controlled substance prescriptions presented simultaneously
  • Cash payments when a patient has insurance coverage
  • Early refill requests or claims of lost medications
  • Altered or photocopied prescriptions

Internal Diversion Warning Signs

Staff diversion may involve unexplained inventory discrepancies, employees working unusual hours near the controlled substance vault, or wasting documentation that lacks proper witnessing.

Prevention Strategies

Inventory Controls

Perpetual inventory systems track every unit of Schedule II medications. Regular cycle counts help identify discrepancies before they become significant losses. Automated dispensing cabinets in hospitals provide electronic tracking of each transaction.

Regulatory Compliance

DEA Form 222 is required for ordering Schedule II substances. All controlled substances must be stored in a securely locked, substantially constructed cabinet or dispersed throughout non-controlled inventory.

Reporting Obligations

Pharmacy staff must report suspected diversion through proper channels. DEA Form 106 is used to report theft or significant loss of controlled substances. Technicians should understand their state board reporting requirements as well.
