Benzodiazepines and Controlled Substance Scheduling

Understand benzodiazepine medications and their DEA scheduling for the PTCB certification exam.

Common Benzodiazepines

Benzodiazepines are Schedule IV controlled substances used for anxiety, insomnia, and seizures. Key medications include alprazolam (Xanax), lorazepam (Ativan), diazepam (Valium), and clonazepam (Klonopin). They enhance GABA activity in the brain, producing calming effects.

Pharmacy technicians must understand controlled substance handling procedures including proper storage, dispensing records, and DEA regulations when working with these medications.

Risks and Alternatives

Benzodiazepines carry risks of dependence, tolerance, and respiratory depression, especially when combined with opioids. The FDA requires a boxed warning about concomitant use with opioids. Flumazenil is the reversal agent for benzodiazepine overdose.

Non-benzodiazepine alternatives like buspirone (BuSpar) for anxiety and zolpidem (Ambien) for insomnia are also commonly tested. Zolpidem is also a Schedule IV controlled substance.
