Antiepileptic Medication Monitoring: A Guide for Pharmacy Technicians

Understand antiepileptic drug monitoring, therapeutic drug levels, and why narrow therapeutic index medications require close attention.

Why Antiepileptic Drug Monitoring Matters

Antiepileptic drugs (AEDs) are among the most commonly monitored medications in pharmacy practice. Many AEDs have a narrow therapeutic index, meaning small changes in blood levels can lead to toxicity or therapeutic failure. Pharmacy technicians must understand monitoring requirements to support safe dispensing.

Commonly Monitored AEDs

  • Phenytoin (Dilantin) - therapeutic range: 10-20 mcg/mL
  • Carbamazepine (Tegretol) - therapeutic range: 4-12 mcg/mL
  • Valproic acid (Depakote) - therapeutic range: 50-100 mcg/mL
  • Phenobarbital - therapeutic range: 15-40 mcg/mL

Phenytoin: A Special Case

Phenytoin displays zero-order kinetics at therapeutic doses, meaning small dose increases can cause disproportionately large increases in blood levels. This makes it one of the most dangerous AEDs to manage without proper monitoring.

Key Considerations for Technicians

Generic Substitution Concerns

Many states have specific regulations about substituting generic AEDs. Because bioavailability can vary between manufacturers, switching products may alter drug levels. Technicians should alert the pharmacist when a manufacturer change occurs.

Drug Interactions

AEDs are notorious for drug interactions. Carbamazepine is a potent CYP3A4 inducer, while valproic acid is an enzyme inhibitor. These interactions can affect the levels of other medications the patient takes.
