340B Drug Pricing Program: A Pharmacy Technician Overview

Learn how the 340B program provides discounted drugs to qualifying healthcare organizations and what pharmacy technicians need to know about compliance and operations.

What Is the 340B Program?

The 340B Drug Pricing Program, established by Section 340B of the Public Health Service Act in 1992, requires drug manufacturers to provide outpatient drugs to eligible healthcare organizations at significantly reduced prices. The program helps safety-net providers stretch scarce resources to reach more patients. Understanding 340B is increasingly important for pharmacy technicians working in hospitals and clinics.

Who Qualifies as a Covered Entity?

Eligible organizations include federally qualified health centers (FQHCs), disproportionate share hospitals (DSH), children's hospitals, critical access hospitals, and certain federally funded clinics. These covered entities register with the Health Resources and Services Administration (HRSA) to participate.

How It Works

Covered entities purchase outpatient drugs at the 340B ceiling price, which is substantially lower than the average wholesale price. The savings can be used to fund patient care programs, subsidize medications for uninsured patients, or support pharmacy operations. Some covered entities use contract pharmacies to dispense 340B drugs to eligible patients.

The Technician's Role

Pharmacy technicians in 340B settings must understand:

  • Patient eligibility - Only patients of the covered entity receiving an outpatient prescription from an eligible provider qualify for 340B pricing.
  • Inventory management - 340B and non-340B inventory must be carefully tracked to prevent diversion and duplicate discounts.
  • Replenishment models - The two main models are the virtual inventory (replenishment) model and the physical inventory (segregation) model.
  • Compliance audits - HRSA conducts audits to ensure covered entities comply with program requirements.

Common Compliance Issues

The most frequent 340B violations include dispensing 340B drugs to ineligible patients, failing to prevent duplicate discounts with Medicaid, and inadequate record-keeping. Technicians play a key role in maintaining accurate records and following standard operating procedures.

While 340B specifics may not dominate the PTCE, understanding the program strengthens your operational knowledge and makes you more valuable in hospital pharmacy settings.
