The Components
OPQRST covers onset, provocation or palliation, quality, region or radiation, severity, and time. Together these questions describe the pattern of the complaint and help distinguish between possible causes.
Applying It
For chest pain, ask what the patient was doing when it started, whether anything makes it better or worse, how it feels (pressure, sharp, tearing), where it radiates, how severe it is on a 0 to 10 scale, and how long it has lasted. Document responses in the patient's own words when possible.
Common Pitfalls
Do not lead the patient toward an answer. Reassess pain after any intervention to check for change. Remember that pain scales are subjective, and a patient who appears comfortable may still have a serious condition.
