Ongoing Assessment
Repeat the primary assessment, reassess vital signs, recheck interventions, and reevaluate the chief complaint. Look for trends: improving, stable, or deteriorating. Make sure oxygen is flowing, bleeding is controlled, and splints remain secure.
Handoff Structure
A strong report includes the patient's age and sex, chief complaint, history of present illness, pertinent history, vital signs, physical findings, treatment given, and response to treatment. Many services use frameworks like MIST or SBAR to keep reports concise.
Tips
Speak clearly, avoid unnecessary detail, and invite questions. Provide written documentation as soon as possible. A good handoff improves continuity of care and reduces errors.
