Exam Guide
The NREMT cognitive exam is administered by the National Registry of Emergency Medical Technicians and is required for EMS certification in most states. The exam uses Computer Adaptive Testing (CAT) and is offered at two certification levels: EMT and Paramedic.

Bleeding control, shock management, musculoskeletal injuries, head/spinal injuries, burns, and multi-system trauma assessment.
Medical emergencies (stroke, diabetes, seizures, allergic reactions, poisoning), obstetric emergencies, neonatal care, and pediatric assessment.
Scene safety, triage, incident command, ambulance operations, documentation, legal/ethical issues, and patient communication.
80 to 150 questions, 2.5 hours, $110
Advanced airway management (intubation, RSI, cricothyrotomy), ventilator management, capnography/ETCO2 interpretation, and respiratory pharmacology.
12-lead ECG interpretation, ACLS algorithms, cardiac pharmacology, pacing, cardioversion, defibrillation, and post-cardiac arrest care.
Advanced trauma assessment, hemorrhage control, chest decompression, fluid resuscitation, burn management, and trauma pharmacology.
Advanced medical assessment, toxicology, endocrine emergencies, neurological emergencies, obstetric complications, and pediatric advanced life support.
Mass casualty incidents, hazmat awareness, research and evidence-based practice, crew resource management, and medical-legal considerations.
EMT: Complete a state-approved EMT education program that meets or exceeds the National EMS Education Standards.
Paramedic: Complete a CoAEMSP-accredited Paramedic education program. Must hold current EMT certification.
Both Levels: Must hold current CPR certification (BLS for Healthcare Providers or equivalent). Program director must verify completion.
The NREMT uses Computer Adaptive Testing, meaning the exam adjusts question difficulty based on your performance. After each answer, the algorithm selects the next question at an appropriate difficulty level. The exam continues until the system has enough information to determine whether you meet the passing standard in each content area.
This means the number of questions varies by candidate. A shorter exam does not necessarily mean you failed, and a longer exam does not mean you are struggling. The algorithm needs sufficient data to make a competency determination with confidence.
You cannot skip questions, go back to change answers, or review previous questions. Each question must be answered before proceeding. The exam is administered at Pearson VUE testing centers nationwide.
In addition to the cognitive exam, NREMT certification requires passing a psychomotor (practical skills) exam. This hands-on assessment evaluates your ability to perform clinical skills on standardized patients or mannequins. Skills tested include patient assessment, airway management, cardiac management, spinal immobilization, and random skills stations.
The psychomotor exam is administered by your state EMS office or approved testing site, separate from the cognitive exam. Requirements and scheduling vary by state.
NREMTprep gives you 2,000+ flashcards, 100 mini exams, 20 full-length practice exams, and Play & Learn games for both EMT and Paramedic certifications. Free tier included.
The NREMT EMT exam has 70 to 120 questions, and the Paramedic exam has 80 to 150 questions. Both use Computer Adaptive Testing (CAT), which adjusts the number and difficulty of questions based on your performance.
The NREMT does not use a fixed percentage passing score. The CAT algorithm determines competency by evaluating whether you consistently answer questions above the passing standard for each content area. You must demonstrate competency across all areas.
The NREMT EMT cognitive exam costs $80 and the Paramedic cognitive exam costs $110. Retake fees are the same. Additional fees may apply for psychomotor skills testing administered by your state.
The NREMT EMT exam covers Airway/Respiration/Ventilation, Cardiology/Resuscitation, Trauma, Medical/OB/GYN, and EMS Operations. The Paramedic exam covers the same areas with advanced scope including pharmacology, ECG interpretation, and advanced airway management.
